Ambiguius Genetalia ……….An Operative Surprise?
04 year old child phenotypically female out of four female sisters, no brother with ear rings, presented in OPD with left inguinal swelling, reducible. Clinically diagnosed as Left Inguinal Hernia and time given for surgery. During surgery surgeon remained astonished and surprised to see the Left healthy Testes. This time we do not have USG abdomen. A difficult decision what to declare and fate of incidental testes, orchidectomy or Orchidopexy? However we called sonologist and he did USG on operation table. He reported as no female organs identified in side abdomen only slit like vaginal canal present. We did orchidopexy and declared as MALE. Recovery remained uneven full. Parents were happy because they do not have male baby. Chromosomal Analysis from AKUH also confirmed as male baby (total 46 XY chromosome genotypically. Now we will plan for next procedures in collaboration with Plastic Surgeon. It was surprising and amazing for family to change sex after long time.
